Home Modifications
Framework for the delivery of Home Modification and Maintenance Services (HMMS).
Home Modification refers to structural changes to the client's home so they can continue to live and move safely around the house.
The aim of the program is to assist frail older people and people with a disability to remain at home by providing affordable home modifications designed to change the home environment to make it safer and easier to carry out tasks and maintain independence.
Assistance may be given to those who fit the HACC target population:

There are two levels to the Home Modifications:
The work is subsidised, however clients will asked to pay a "client contribution" towards the work performed.
Home maintenance refers to assistance with the maintenance and repair of the person's home, garden or yard to keep their home in a safe and habitable condition.
Home maintenance may include minor dwelling repairs and maintenance such as:
The aim of this service is to provide affordable and basic home maintenance to the HACC target population.
Clients will be asked to pay a client contribution towards the work performed.
A comprehensive assessment of the client's home by an Occupational Therapist is required before Home Modification and some maintenance services are provided.
